If you have already contacted SSLDC staff and are ready to enroll your child, please select the button below for the form/s you will need to fill out. Once your forms are completed, a staff member will contact you about placement.


  • Create one user name and password to use for all children in your family, then enter user name and password into the “New User” registration fields for each child.

  • Complete all fields relevant to your child.

  • These forms are legal documents. Enter your full name in the electronic signature field at the bottom. This is a legal signature. Enter the current date.

  • Click the Save Progress button at the bottom if you want to save and return later to complete the form.

  • After entering all information appropriate to your child, you MUST click on the SUBMIT button for your document to be legally submitted.

  • If you want to register another child, sign-in again with the user name and password you created, then click the Start New button and follow the instructions above.

  • If you want to update information or register your child for another year, sign-in again and click the Edit button on the row with the child’s name, then make any changes. Update the date field next the the signature field and click the Submit button.

to enroll your child, you will need:

  1. All required forms completed

  2. Updated physical and shot record

  3. Copy of your child’s birth certificate

  4. Copy of your photo ID
